About Impact Healthcare
Impact Healthcare began in San Francisco in 2014 with 32 team members and 89 patients. We now have locations across four states (Arizona, California, Oregon, and Utah) and serve thousands of patients in their homes. At Impact Healthcare, we have a vision. And a purpose. Our Services
A serious illness can raise so many questions for the patient and the entire family. Our hospice care team provides answers to these questions while addressing physical, emotional, and spiritual care needs.
Our at-home services, such as nursing, therapy, and support, help patients recover with independence in the comfort of their own homes.
The Bridge to Hospice program offers care coordination for patients and families dealing with end-stage chronic or life-limiting illnesses, enabling a smooth, timely, and seamless shift from home healthcare to hospice care if needed.
Our at-home caregiving allows a person’s needs to be met in the comfort of their home with individualized care plans tailored to them.
